In need of an athletic physical for the 2025-26 school year?  NOW and early summer are GREAT times to help  students get their annual physicals.  The Health & Wellness Centers are OPEN YEAR-ROUND inside 2 PSD locations:

Centennial High School

High quality physical and mental health services are provided by licensed professionals including Nurse Practitioners/Physicians Assistants and licensed Mental Health Workers to any PSD student who enrolls for care.

No co-pays/deductibles are required.  We accept most private insurances, including Medicaid/CHP+.  We offer fee-for service and  a deeply discounted sliding fee schedule for qualifying families. No student enrolled for our care is ever turned away fo9r an inability to pay for services provided at the center.

Services Include:

*Full Primary medical care

*Annual Well-child exams/sports physicals

*Immunizations and health screenings

* Diagnosis/treatment for colds/flu, infections, sprains, wounds, rashes, asthma

* Rapid in-house tests

*Prescriptions

Mental Health Services

*Counseling for mental health issues

*Skill development and resiliency building

*Validated screenings & treatment for depression, anxiety, substance use & more
